The certificate holder must inform MPS-ECAS in writing and in a timely manner of any intended changes in a business unit and/or products and/or processes that are related to the certification scheme and covered by the certification agreement.

This also applies to significant events that may impact the certified management system, such as legal prosecution or emergencies.

By using this form, you can notify MPS-ECAS of your change, after which the modification will be processed.

If additional investigation or information is required, MPS-ECAS will contact you accordingly.

If (for example, during an audit) it appears that the name and/or identity of a (prospective) certificate holder has changed compared to the signed quotation and/or a previous agreement with MPS-ECAS, this document must be completed by the (prospective) certificate holder.

The reason this document must be completed is to ensure it is unmistakably clear with which company MPS-ECAS has an agreement.
The (prospective) certificate holder must always attach documentation to this change form demonstrating that the change has taken place.
Finally, MPS-ECAS must provide written approval for the transfer related to the certification.

If business activities certified by MPS-ECAS have been transferred to or incorporated into another legal entity, the transfer must be approved by MPS-ECAS.
